Signup Questions
Use this page to choose what people are asked when they join online — date of birth, experience, goals, injuries.
Quick steps
- Go to Memberships, click More Actions (⋮), then Signup Form. The Membership signup tab is open.
- On an empty form, click Start with the usual gym questions to add six common ones — or click Add Question to write your own.
- Type the Question and pick an Answer type.
- Pick where to Save the answer to on the customer's record, such as Date of birth.
- Tick Must be answered if it's required, then click Add Question.
- Use the ↑ and ↓ buttons in the list to put the questions in order.
Common questions
Are these asked when I enrol someone at the counter? No. They are only asked when somebody joins online. Fill anything else in on the customer's record.
Can I stop asking a question without deleting it? Yes. Edit it and untick Show on the form. It shows as Hidden in the list.
If I delete a question, do I lose the answers? No. Answers already collected are kept.
Where do the answers show up? In the Profile & Membership section of the customer's record.

The Signup Form page decides what somebody is asked as they join online. Without it, an online signup gives you a name, an email and payment details — everything else has to be asked at the door and typed in by hand.
Open it from Memberships → More Actions (⋮) → Signup Form, or with the Signup Form button on the Membership Plans page.
The Forms
Tabs across the top choose which form you are editing. Each keeps its own questions.

Starting From Scratch
An empty form offers a starting point rather than a blank page. Start with the usual gym questions adds six:
- Date of birth — a date, saved to the customer's date of birth, required.
- Age group — a dropdown of age bands.
- Previous experience / belt rank — short text, with the hint "First time? Just say so."
- What is your top priority? — buttons: Physical fitness, Self-defence, Social involvement, Competitive goals, Having fun.
- Health concerns / injuries — a paragraph, with the hint "Anything we should know about before you train."
- How did you hear about us? — short text.
On the Waiver health questions tab the button reads Start with the usual health questions instead and adds seven yes/no medical questions.
Loading the defaults never duplicates a question you already have. Edit or delete any of them afterwards — they are ordinary questions.

Adding a Question
Click Add Question.

Where the answer is saved

Answers appear in the Profile & Membership section of the customer's record — mapped answers in their own boxes, everything else listed under the question you asked.

The list shows Order, Question, Type and Saved to. Required questions carry a red asterisk; hidden ones carry a Hidden badge.
- ↑ and ↓ swap a question with the one above or below it. That order is the order people see.
- Edit changes any field.
- Delete asks Remove this question? Answers already collected are kept.

Things Worth Knowing
- Renaming a question changes where new answers are filed. Answers that are not mapped to a customer field are stored under the question's wording, so a rename starts a new heading and the old answers stay under the old one. Mapped questions are unaffected.
- Required means required everywhere the form is used. The public signup refuses to continue until every required question is answered.
- The questions are not asked when staff enrol somebody. Enrolling from the admin asks only for the member, the plan and a trial length. Fill the rest in on the customer's record.
- Answers are recorded before payment. Somebody who fills the form in and then abandons the payment screen still leaves you a real customer record with real answers.
- A returning member sees their previous answers already filled in, but only when they are signed in — an email typed into the form never prefills anything.

Best Practices
- Ask the least you can get away with. Every extra question is a chance for somebody to abandon the signup.
- Map anything you will want to search or report on. Date of birth, age group and experience are far more useful on their own fields than as free text.
- Use Hidden rather than Delete for a question you may want back next season — deleting loses the wording, though the answers already collected are kept.
- Put the health question last. It is the longest to answer and the easiest to abandon on.
